| التصوير الطبي
نظام الباكس: PACS
و كلمة PACS هي اختصار ل Picture Archiving & Communication System
التعريف: هو نظام حاسب آلي متكامل يقوم بتخزين وتوزيع وعرض الصور الطبية (مثل صور الأشعة) وهو متكامل مع الأجهزة الرقمية لأخذ الصور وعرضها، وهو كنظام يرتبط مع الأنظمة المعلوماتية الطبية الأخرى كأنظمة المستشفيات HIS و نظام إدارة معلومات الأشعة RIS Hospital Information System " HIS " " RIS " Radiology Information System اِن نظام أرشفة وتناقل الصور الإشعاعية يساعد أي طبيب في المستشفى علىمعاينة صور الأشعة وتقاريرها لأي مريض ومن أي مكـــان بالمستشفى، كما أصبح بالامكان ان يشاهد الصور hكثر من طبيب ومن أكثر من مكــــــان في الوقت ذاته. ان فكرة هذ النظام تعتمد على تقنية الصور الرقمية للأشعة والتي يتم حفظـــــها بصورة الكترونية واسترجاعها مرة اخرى بسرعة فائقة لاتتجاوز الثواني المعدودة، مقارنـة بالطريقة السابقة لحفظ ملفات الأشعة والتي تتطلب مساحات كبيرة لحفظ ملفات الاشعة ووقتاً طويلاً لاستردادها وارسالها للعيادات واجنحة المرضى. وهذا بدوره خفــــــض معاناة الاطبـاء والممرضين في انتظار وصول الأشعة والذي كان يؤدي دوماً الى تأخير تقديم الخدمة العلاجية للمرضى.
و من فوائد هذا النظام الجديد امكانية حفظ صور الاشعة لعشـــرات السنين في مساحـــــات صغيرة ( لاتتجاوز غرفة مساحة غرفة عادية) مقارنة بالنظام الســـــابق والذي بموجبه كان المستشفى يضطر الى اتلاف الافلام التي يمضي عليها اكثر من خمس سنوات حسب نظـــــام وزارة الصحة المعمول به في كل المستشفيات لعدم توفر مساحة للتخزين. اضافة على ذلــك أصبح بالامكان ادخال أي صورة اشعة قديمة سواء من داخل المستشفى او خارجه الى نظام الارشفة عن طريق الماسح الضوئي ومن ثم حفظها في النظام ومقارنتها بالصور الجــــديدة
وهي تتكون من عدة أجزاء أساسية 1- Computers / servers سيرفرات (خوادم)
2- Communication Network & Digital infrastructure شبكة اتصال وبنية تحتية رقمية
3- Storage Media unit وحدة وسط تخزين
4- Data Acquisition Conroller متحكم جمع المعلومات
5- Display Workstations محطات عمل لمشاهدة الصور
6- Image data compression برنامج لضغط الصور
7- Artificial intelligence ذكاء اصطناعي
8- Opto-electronic devices أجهزة الكتروبصرية
9- Software برنامج متخصص
أنواع الباكس: الباكس الصغير Mini PACS وهو النوع الذي تخدم فقط داخل قسم الأشعة ولا يستطيع تصدير الصور الى خارج قسم الأشعة
الباكس المتوسط Mid Scale PACS وهو النوع القادر على إخراج الصور الى عدد محدود من الأماكن خارج قسم الأشعة مثل (العناية المركزة، غرف العمليات)
الباكس الكبير Large Scale PACS وهو النوع الذي يستطيع اخراج وتوزيع الصور لأي نقطة داخل وخارج قسم الأشعة ..... يعني ممكن تشوف الصور من أي جهاز كمبيوتر موصول بشبكة المستشفى سواءا بالعيادات الخارجية أم العناية المركزة أم الطوارئ أم غرف المرضى و محطات التمريض ..الخ وكذلك يتمكن طبيب الأشعة من مشاهدة الصور عن بعد بواسطة الانترنت (اذا توفرت هذه الخاصية)
وهذا النوع الكبير هو الذي سيتم التركيز في الكلام عليه
والنظام يعمل كالتالي:
عند مكتب الإستقبال الخاص بالأشعة أو مكتب المواعيد يتم استلام طلب الأشعة الكترونيا مباشرة من الطبيب المعالج
يقوم الطبيب بإنشاء طلب الكتروني عن طريق نظام HIS (نظام معلومات المستشفى) ومن ثم يتم تحديد موعد للمريض أو تسجيل عمل اجراء فوري للمريض Walk-in
و قبل أخذ أي صورة أشعة من أي مصدر (سواء جهاز أشعة سينية أم أشعة مقطعية أم رنين مغناطيسي أم موجات فوق صوتية ...الخ) يتم جلب المعلومات الأساسية الخاصة بالمريض من RIS والذي بدوره يأخذ المعلومات من HIS وتعرف هذه المعلومات ب المعلومات الديموغرافية للمريض مثل رقم الملف الطبي و الإسم والسن والجنس والعمر وهكذا
طبعا بعد الفحص يتم ادخال اسم الفني والطبيب المسؤول عن قراءة الصور ويتم كل ذلك بشكل آلي (طبعا اذا كانت مبرمجة بطريقة صحيحة) بالإضافة على القدرة على إدخال بعض المعلومات يدويا كإستخدام الصبغة وكم كميتها ونوعها.... الخ
عند اكتمال ادخال معلومات واكتمال عمل الفحص يتم دفع الملف الذي يحتوي على الصور الاشعاعية والمعلومات المصاحبة له الى وحدة الأرشيف بنظام الباكس PACS Store ومن ثم يمكن لأي نقطة موصولة بالشبكة الحصول على الصور فور اعطاء أمر الإستدعاء أو يتم ارسال الصور مباشرة للمحطة التشخيصية Diagnostic Workstation
وعادة ما يتم استدعاء الصور من قبل اطباء الأشعة داخل قسم الأشعة وهم يحصلون على صور كاملة الحجم دون ضغط وهذا مايعرف ب Diagnostic Quality Images
ويتم مشاهدتها في محطات مشاهدة وتشخيص عالية الجودة Diagnostic Quality Review Workstations
أما بالنسبة للأطباء خارج قسم الأشعة خاصة الموجودين في الطوارئ والعناية المركزة والعيادات فيمكنهم الحصول على صور مضغوطة ولكنها جيدة ومعقولة الحجم عن طريق محطات جيدة الجودة تعرف بـ Review Workstations

| قسم العلوم الإشعاعية
العلوم الإشعاعية هو ذلك الفرع من العلوم الصحية الذي يعنى باستخدام الأشعة السينية وأشعة جاما والذبذبات فوق الصوتيه والرنين المغناطيسي في تشخيص الأمراض وعلاجها. وكثيراً ما يتطلب من المتخصص في هذا المجال أن يجري الكشوفات الإشعاعية في غرفة الطوارىء والعمليات أو في موقع المريض في سريره. كما أن مسئوليته تتضمن تحضير المريض لفحوصات معينة والتأكد من كفاءة عمل الأجهزة التشخيصية المستعملة.
ويهدف برنامج العلوم الإشعاعية إلى إعداد الطلاب بالأسس العلمية العامة والخلفية التعليمية والمهارات اللازمة للممارسة بنجاح في مجال تقنية العلوم الإشعاعية وتدريبهم في التصوير الإشعاعي التشخيصي والطب النووي والذبذبات فوق الصوتية والرنين المغناطيسي والتصوير الإشعاعي الطبقي بإستخدام الحاسب الآلي والعلاج الإشعاعي وتمكنهم من العمل مع أطباء الأشعة وأطباء الطب النووي والأطباء المتخصصين في فحص الأورام والقرح والتغييرات المرضية والكسور وعطل أعضاء الجسم المختلفة.
ويعطي هذا البرنامج عناية خاصة للطرق التي تؤكد سلامة المرضى والأفراد الآخرين من التأثيرات الإشعاعية. ويؤهل البرنامج الخريجين للعمل كأخصائيين في مجال العلوم الإشعاعية الخاصة بتقنية التشخيص والعلاج الإشعاعي في أقسام الأشعة والتصوير الطبي في المستشفيات والمراكز الصحية الأخرى | |

| Ultrasound Ultrasound was the first nonradiologic technique used to image the body. Ultrasound in radiology stems from the development of pulse-echo radar during World War II. First used to detect defects in metal structures, ultrasound, or sonography, became a useful diagnostic tool in the late 1950s and early 1970s. As its name suggests, ultrasound uses sound waves rather than electro-magnetic radiation to image structures.
A common use of ultrasound is to provide images of a fetus. A sound transmitter is used to send waves into the body from various angles. As these waves bounce back off the uterus and the fetus, they are recorded both on a television screen and in a photograph. With the more advanced Doppler ultrasound, this technology can be used for everything from imaging atherosclerotic disease (the thickening of arteries) to evaluating the prostate and rectum.

| Computers And The New Era Of Radiology

Except for ultrasound, from the day Roentgen discovered the x ray until the early 1970s, radiology relied solely on the application of x rays through refined radiographic techniques. These applications were limited by the x ray's ability to discern only four different kinds of matter in the body: air, fat, water (which helps make up tissue), and minerals (like bone). In addition, while the x ray images bone well, it cannot image what lies behind the bone unless angiography is used. For example, a standard x ray could reveal damage to the skull but would not reveal tumors or bleeding vessels in the brain unless they calcified or caused changes to the skull. Although the development of angiography allowed scientists to view the arteries in the brain, angiography is somewhat painful for the patient and does not reveal smaller but still serious tumors and lesions.
The high-tech era of radiology coincided with rapid advances in computer technology. By using computers to analyze and interpret vast quantities of data, scientists began to develop new and better ways to image the body. Imaging processes like computed tomography, positron emission tomography, magnetic resonance imaging, and single photo emission computed tomography all rely on the computer. With these techniques, radiologists are able to diagnose a wider range of diseases and abnormalities within the body. | |

Although still relying on the x ray, this radiographic technique uses a computer to process the vast amount of data obtained from an electronically detected signal. This information is scanned into a digital code which the computer can transform into a video picture. Although magnetic resonance imaging (MRI) dates back to 1946, it was used primarily to study atoms and molecules and to identify their properties. In 1978, the first commercial MRI scanner was available, but it was not until the 1980s that MRI became a useful tool for looking into the human body. MRI works by using a huge magnet to create a magnetic field around the patient. This field causes protons in the patient's body to "line up" in a uniform formation. A radio pulse is then sent through the patient, which results in the protons being knocked out of alignment. When the radio pulse is turned off, the protons create a faint but recordable pulse as they spin or spiral back into position. A computer is used to turn these signals into images.
This nonradiological technique has many benefits. It does not use ionizing radiation, which can be harmful to humans. In addition, it has superb low-contrast resolution, allowing radiologists to view and diagnose a wider range of diseases and injuries within the patient, including brain tumors and carotid artery obstructions. Positron emission tomography (PET) and single photon emission computed tomography (SPECT) are two more technologies that rely on computers. PET has been used primarily to study the dynamics of the human body. In other words, not just to see images, but to understand the processes that go on in certain areas of the body. For example, radioisotopes (naturally occurring or artificially developed radioactive substances) injected into a patient can be imaged through PET computerized technology, allowing scientists to watch how metabolism works in the brain and other parts of the body. With this technology, scientists can watch glucose metabolism, oxygen consumption, blood flow, and drug interactions.
SPECT uses radionuclides (radioactive atoms) to produce images similar to CT scans, but in much more precise three-dimensional images. The use of dual cameras, one above and one below the patient, enables radiologists to obtain simultaneous images that are then processed by computers to provide improved resolution of a structure in less time. PET/CT is a nuclear medicine technique which combines, in a single gantry, a positron emission tomography scanner & an XRAY computed tomography scanner , to acquire sequential images from both devices in the same session, which are combined into a single superposition image
PET/CT يعتبر التصوير المقطعي بالإصدار البوزيتروني هي تقنية تصوير في الطب النووي تبين صور ثلاثية الأبعاد لبعض أعضاء الجسم وما قد يكون فيها من ورم سرطاني أو نقيلات سرطانية، كما يمكن بواسطتها تفقد مختلف العمليات الوظيفية في الجسم، مثل العمليات الحيوية للجهاز الهضمي.

Interventional radiology is one of the more recent developments in radiology.
